What information does the worksheet request?

The fields cover vehicle year, make and model, mileage, the closest service topic, observed symptoms, recent work and questions. Enter only the details you want in your own note. The tool is optional and does not verify a diagnosis or appointment.

Does the worksheet transmit or store the fields?

The worksheet itself does not transmit your entries. It does not place the entered fields in local storage, session storage or cookies. Reloading or leaving the page can clear the draft, so download or copy it first if you want to keep it.

What happens when you download or copy?

The Download action creates a plain text file on your device. The Copy action asks the browser for clipboard access. After either action, you decide where the note is saved or shared.

What happens after you leave the worksheet?

Phone and email links open your device's apps. Any information you choose to send after leaving the worksheet is handled by the recipient or external platform under its own privacy practices.

Who controls this site?
